LSD
Chemicky se jedná o diethylamid kyseliny d-lysergové.Látka byla poprvé syntetizována v roce 1938 v laboratořích firmy Sandoz. V roce 1943 se díky neopatrnosti při resyntéze Albert Hoffman intoxikoval a objevil tak její účinek. Otcem nebo prorokem LSD byvá zván Timothy Leary, který se zasloužil o popularizaci LSD.
Mechanismus účinkuMechanismus účinku LSD závisí na ovlivnění serotoninových receptorů (Receptor je jakýsi vypínač, který může být zapnut/vypnut/zablokován určitou látkou; tím dochází k modulaci vlastností cílové buňky.) v mozku. LSD patří mezi agonisty S2 recetporů (tj. látka, která aktivuje S2 recetpory), tzn. má psychotické účinky (může vyvolat psychózu).
LSD je velmi účinná látka.
Pro intoxikaci dostačují dávky kolem 50 gama (0,000 05 gramu).
· Dávky do 100 gama vyvolávají většinou euforii, někdy úzkosti. Realitu vnímá uživatel jen mírně zkresleně, je schopen se v situaci orientovat.
· Dávky do 250 gama vyvolávají značnou ztrátu kontaktu s realitou, stav je podobný psychóze.
· Od 200 gama výše jsou dávky, které označujeme jako psychedelické. Kontakt s realitou je zcela porušen. Dochází ke ztrátě orientace v sobě samém i ve svém okolí.
Vzhledem k minimálním množstvím, která mohou vyvolat intoxikaci, je LSD většinou spojeno tzv. matricí, do které je napuštěno (kupříkladu jako trip se označuje papírek napuštěný LSD o rozměrech 5x5 mm).
Otrava se projeví po užití per os (ústy) za 1/2 až 3/4 hodiny, vrcholí asi za 3 hodiny, pak je asi 2 hodiny stabilní. Účinek odeznívá po 6 - 8 hodinách. Někdy může dojít během 24 hodin k druhému nástupu příznaků.
Nástup intoxikace se většinou ohlásí pocity závratě, nevolnosti, někdy zvracením. Objevuje se slinotok, kuřáci mívají tendenci kouřit. Somaticky se objevuje rozšíření zornic, stoupá krevní tlak a tep, stoupá teplota, intoxikovaný se potí.
Psychické projevy jsou různé. Vědomí je zachováno, bývá porušena kontinuita prožívání. Je oslabena kontrola vůle a kritičnost, taktéž časová orientace. Pro intoxikaci je klíčový tzv. set a seting - tj. vnitřní stav intoxikovaného a zevní prostředí. Reakce je výsledkem spolupůsobení látky, psychologického a fyzikálního okolí, osobnostní struktury pokusné osoby, terapeuta a očekávání, co by látka mohla dělat. Mění se prožívání emocí, většinou převládá eufroie a hypomanické tempo, může ale také převládat panická hrůza a bezbřehá úzkost. Intoxikace bývá provázena poruchou pohybové koordinace, alternace vnímání je značná, hlavně zrakového a tělového. Změněna je perspektiva, odhadování vzdáleností, barvy. Vyskytují se četné halucinace (resp. pseudohalucinace), tělové se projevují změněným vnímáním tělesných proporcí. Pocity mohou být někdy velmi nepříjemné, provázené úzkostí. Intoxikovaní líčí snové krajiny, hovoří s klobouky, slyší barvy atd. Časté bývají obavy ze ztráty duševního zdraví, někdy k tomu skutečně dojde a stav přetrvává dlouhodobě.
Rizika spojená s užíváním jsou poměrně značná. LSD nepatří mezi drogy vhodné k užití "pro zábavu", pro svou razanci a předem nevypočitatelný efekt. Nezkušeného uživatele může stav překvapit. Trvalé psychické následky či sebevražedné pokusy nejsou vyjímečné. Bezpečnost může zvýšit průvodce, který "převede" uživatele přes úskalí intoxikace. Stává se, že přetrvává vystašenost, zmatenost, pocity "otevření" hlavy.
Specifickým rizikem typickým pro skupinu halucinogenů je "bad trip". Jedná se o mimořádně nepříznivě probíhající intoxikaci s hrůznými obrazy, pocitem vitálního ohrožení. Intoxikovaný pak může být nebezpečný sobě i svému okolí. Uklidnění intoxikovaného, vyvedení z prostředí, které vnímá jako rizikové, je nezbytné. Doporučuji také všem, kteří "bad trip" prožili, aby se dalších intoxikací raději vystříhali.
Specifickým rizikem intoxikace LSD jsou tzv. "flashbacky" (v překladu záblesky paměti). Jedná se o návrat stavu prožitého při intoxikaci s určitým časovým odstupem. Obyčejně literatura uvádí 1 rok, může to však být i déle. Flashbacky bývají často krátkodobé, několikaminutové či vteřinové, průběh však může být co do délky a intenzity shodný s intoxikací. Flashbacky se někdy objeví jednorázově, jindy přicházejí opakovaně. Stav může vzniknout zdánlivě bez příčiny, pravděpodobně se jedná o reakci na vyvolávající podnět, či soubor vyvolávajících podnětů, které stav vyprovokují. Jako podnět se může například uplatnit modelová situace, ve které docházelo k intoxikaci. Podnět však může zůstat skrytý.
Riziko závislosti je poměrně malé. Obyčejně nedochází ke ztrátě kontroly, jako je tomu například u závislosti na alkoholu. Časté jsou po opakovaných intoxikacích změny sebepojetí a sebehodnocení. Někdy se objevuje přetrvávající vztahovačnost, úzkost, častá je ztráta kontaktu s realitou. Uživatelé se vztahují ke světu až s jistým "filozoficky moudrým" náhledem, pro normální život však bývají často nepoužitelní.
V souvislosti s LSD se často mluví o poškození genetické informace lidských buněk. Pro vznik poškození není údajně důležitý počet intoxikací, ale jen to, zda intoxikace proběhla. To znamená, že k poškození dochází již po první intoxikaci, další už na kvantitu poškození (chromozomových aberací) nemají statisticky významný vliv. Kombinace s dalšími halucinogeny zvyšuje pravděpodobnost poškození. Novější práce riziko aberací nepotvrzují, pro praxi je ale důležité, že užívání drog je spojeno s celkovým životním stylem, který se může uplatnit jako vyvolávající faktor nebo kofaktor. Je také možné, že mutagen obsahuje matrice, ve které je droga navázána.
Charakteristika látky:
1. Účinné dávky jsou již od 0,000 05 gramu, jedná se tedy o látku mimořádně účinnou.
2. Svým primárním účinkem vyvolává psychotický stav.
3. Nepatří mezi "rekreační" drogy, intoxikace může mít vážné psychické následky.
4. Dlohodobě i po jednorázovém užití drogy může dojít k návratu stavu prožitého při intoxikaci.
5. Jedná se o látku s potenciálně mutagenním účinkem.
6. Látka nevyvolává závislost, při častém užívání dochází k osobnostním změnám.
